Office Hours

We see patients Monday through Friday from 8:30am to 5:00pm. Same day appointments are usually available.

Your Appointment

Patients are seen by appointment, helping to ensure minimal wait time. Please call our office in advance at (904) 730-5158. Same day appointments are usually available and walk-ins are welcome for drug and alcohol testing.

Although we make every effort to stay on schedule, we ask for your understanding should emergencies and complicated visits occasionally cause delays. In these cases, our staff will keep you informed, giving you the option of rescheduling your appointment. You can help us by:

• Informing the receptionist about the nature of your visit when you make your appointment, so adequate time may be allowed for the visit
• Bringing your driver's license or picture ID
• Notifying the office at least 24 hours in advance if you are unable to keep your appointment

New Patients

If you are a new patient, please arrive at least 30 minutes early to complete the registration process. You may also download and complete our patient registration forms and bring them with you when you come in.
